BLUE BADGE QUESTIONNAIRE
You may be eligible for a Blue Badge, however, your local council will require you to provide evidence that you qualify for a Blue Badge.
As your application will be based around your walking ability, a mobility assessment from our Occupational Therapist will support a successful application.
You can complete your application for yourself via the link below which will direct you to the Gov.uk website, however without supporting evidence you application may take longer for the council to assess your needs or be rejected. We wish you well with your application....
*Please note that a mobility assessment is not required if you automatically qualify for a Blue Badge
